- An inspection every 24 hours in service on the regulation's schedule
A driver vehicle inspection report template set up for Manitoba, with fields for the carrier's name, licence plate or unit number, odometer reading (if equipped), time of inspection, urban municipality or highway location, person who performed the inspection (printed name), height and width of the vehicle and load and defects found. In Word and PDF.
Set up for Manitoba
- The person who inspects for the Schedule A, B, C or E defects prepares a legible written or electronic report without delay with the licence plate or unit number, the odometer reading if there is one, the carrier's name, the date and time, the urban municipality or highway location, and the inspector's printed name, plus a statement signed by the inspector and by the driver that the vehicle was inspected under the regulationCommercial Vehicle Trip Inspection Regulation, M.R. 95/2008, s. 12 (1) (a), (d)
- Every report records the height and width of the vehicle and load, and when a load is not entirely inside the body, the driver measures and records its height and width, covering every such load carried since the last inspectionM.R. 95/2008, ss. 7 (7), 12 (1) (b)
- The report says when no minor or major defect was detected, records every minor or major defect detected, and is never false or misleading about a defect or about when, whether or by whom it was repairedM.R. 95/2008, ss. 15, 16 (1), 23
Also required in Manitoba
These rules are about how and when you report, not the template's fields. Check them before you deliver.
- Trucks and tractors with a GVWR of 4,500 kg or more not used solely for personal transportation, their trailers of 4,500 kg or more, buses carrying passengers for compensation and school buses are inspected every 24 hours in service on the applicable schedule (A trucks and trailers, B buses, C and D motor coaches, E school buses), and again before use after more than 24 hours out of service; the carrier makes sure of itM.R. 95/2008, ss. 1, 3, 7, 11
- The driver reports a minor defect to the carrier before the next required inspection and a major defect without delay, monitors the vehicle while driving and records any new defect; a major defect is repaired before the vehicle is next driven, and a minor defect before the next scheduled inspection, after which the vehicle may only go straight to the nearest repair facility if safeM.R. 95/2008, ss. 12 (4), 17, 18, 19 (am. M.R. 107/2024)
- The driver carries the current inspection report and the applicable inspection schedules while driving and produces them to an inspector on demand; an inspector who is not the driver hands the report to the driver, who signs it at once and is then deemed aware of the defects recordedM.R. 95/2008, ss. 6, 12 (5)-(6), 13, 14
- The driver gives the report to the carrier within 20 days (40 days for a Schedule D report), and the carrier keeps the original reports and any certification of major defect repairs for at least six months and produces them on demandM.R. 95/2008, ss. 21, 22
- An electronic report holds the same information as a written one, can be produced at once on screen, on paper or in another intelligible form, logs every disconnection and reconnection, and any printed copy is signed on each page by the driver; the carrier also keeps blank forms in the vehicleM.R. 95/2008, s. 12 (3)
- Not covered: two- or three-axle vehicles hauling the driver's or employer's own primary farm, forest or aquatic products, snow removal vehicles of a traffic authority, Winnipeg and Brandon transit, towed drive-away units, emergency use, and limited-use vehicles under 6,800 kg kept in Manitoba; for a vehicle other than a school bus that is operated that day only within 160 km of the terminal where the driver reports, or a sole owner-operator's only vehicle, the duties to prepare, record defects in and carry the report and to carry the schedules do not apply, though the daily inspection still doesM.R. 95/2008, s. 2
